Supplying information is voluntary, however if you cannot provide or do not wish to provide information sought that it may not be possible to take therapy forward. The personal data you or your referrer shares with PSYCHTHRIVE LTD will be stored for the purposes of the therapy process and in your best interests. This will include storing contact details on electronic devices such as mobile phones.
Your personal data and information will not be shared outside of our Clinicians' database unless they think there is an imminent risk to your safety or the safety of others or it is specifically agreed to do so with your therapist e.g. sharing information with a family relative or with your GP at your request.
When confidentiality has to be breached, it will be discussed with you first but only if this is possible in a timely manner to ensure safety. Your personal data will be stored responsibly and confidentially to the standards of the General Data Protection Regulation (GDPR, 2018).
Any process notes of confidential information or personal data for the purposes of the therapists personal notes or processing (e.g. session notes) will be stored for 6 months following completion of your final session and then destroyed securely. Any formal records (e.g. formal reports or therapy letters) containing your personal confidential information will be stored for seven years following completion of your final session and then destroyed securely. The only record that will retained indefinitely will be a consent form.
Once the time periods for storing written and electronic personal data has passed, your personal information will be deleted or destroyed appropriately in line with GDPR expectations. You have the right to request that your confidential personal data is destroyed/deleted at any time (please note that this would potentially jeopardise your right to make a claim against the therapist at a later date).
You will not record sessions without the express permission of the therapist. Content of therapy sessions and any personal opinions about yourself or your therapist will not be shared without consent or shared on social media of any kind by you or your therapist. You can withdraw your consent at any time and that withdrawal of consent may mean therapy sessions cannot continue.
